I authorize C&J Equipment to verify the accuracy of the information I have provided and to perform background checks which include, but are not limited to, criminal background checks, reference checks, and employment verifications. I release from all liability anyone supplying such information and I also release the Company from all liability that may result from making an investigation.

If hired, I agree to abide by all the Company's rules and regulations and I understand that, if employed, my employment may be terminated with or without notice or cause at any time at the option of either the Company or me.
